Strength Training for Perimenopausal Women on GLP-1 Medications

I want to establish two things before we start.

First, I'm not a doctor. My background is in business, and I spent seven years as a CrossFit Level 1 Coach. More importantly, I have spent 30 years as a husband and father.

My wife is a strong woman, an executive, perimenopausal, and using a GLP-1 medication.

I watched the fitness industry struggle to give her useful answers.

Generic programs. Apps that didn't know she was in perimenopause. Training plans that couldn't account for days when medication side effects changed what she was capable of doing.

The second is weight loss.

When someone loses a substantial amount of body weight, that weight does not come exclusively from body fat. Some lean tissue can be lost as well.

That matters for women using GLP-1 medications because these medications can produce substantial weight loss.

The goal shouldn't simply be to make the number on the scale smaller.

We should also be asking:

How much strength and lean mass are we preserving along the way?

Resistance Training Matters

Resistance training provides the body with a reason to retain and build muscle.

It also supports strength, physical function, and bone health — all things that become increasingly important as we age.

It means resistance training should be treated as a meaningful part of the health and fitness strategy rather than an optional addition after cardio.

And it needs to be progressive.

A weight that was challenging six months ago may no longer provide the same training stimulus today. Maintaining strength and muscle requires sufficient resistance and progression over time.

"Lift heavy" also needs some context.

Heavy is relative to the person.

For one woman, that might mean a barbell squat. For another, it might mean a challenging leg press, goblet squat, or bodyweight squat. There is no hierarchy here. The right exercise is the one that provides an appropriate challenge for you, given your experience, ability, equipment, and current capacity.

The objective isn't to hit an arbitrary percentage of a one-repetition maximum.

The objective is to provide an appropriate resistance-training stimulus that can progress as the person gets stronger.

GLP-1 Medications Change the Training Context

This is where generic programming starts to become a problem.

GLP-1 medications can be associated with side effects including nausea, vomiting, gastrointestinal discomfort, fatigue, and reduced appetite.

Those effects don't happen identically for every person or every day.

A woman may feel completely normal during one training session and considerably worse during another.

That creates a programming problem.

A static training plan assumes that today's capacity resembles the capacity the program predicted when it was written.

Sometimes it does.

Sometimes it doesn't.

There isn't one universal protein target that is appropriate for every woman.

Body size, age, activity level, training volume, overall energy intake, health status, and individual goals all matter.

But if maintaining muscle is one of the objectives while losing substantial body weight, protein intake deserves deliberate attention.

For some women, that may simply mean prioritizing protein-rich foods.

For others, a protein supplement may be a practical way to help meet nutritional needs when appetite is limited.

Perimenopause Adds Another Layer

Perimenopause isn't one predictable physiological state.

Cycles can become irregular.

Sleep can change.

Two women can both be 48 and perimenopausal and arrive at the gym with completely different training capacity.

Knowing how she is responding to it today matters more.

Don't confuse adaptation with failure.

Changing a workout because you're experiencing significant fatigue, nausea, poor sleep, pain, or another meaningful change in readiness isn't "giving up."

Sometimes reducing the session is appropriate.

Sometimes changing the exercise is appropriate.

Sometimes recovery is appropriate.

The goal is consistency over months and years, not winning one workout.

Talk to your healthcare team.

Your prescribing clinician should know about meaningful medication side effects and significant changes in your ability to eat, hydrate, exercise, or recover.

A registered dietitian can also help determine appropriate energy, protein, and nutrient intake during significant weight loss.

Training belongs alongside that care, not in competition with it.

Why This Matters

Weight loss is easy to measure.

You step on a scale and get a number.

Strength, muscle retention, physical capacity, and what your body will be capable of doing ten or twenty years from now are harder to see.

That doesn't make them less important.

For women navigating perimenopause while using GLP-1 medications, we should be thinking about both sides of the equation:

Lose what you want to lose while protecting what you want to keep.

That's a much better objective than weight loss alone.

And it's exactly the kind of problem adaptive training should be built to solve.

Some days may only require modifying the duration, volume, intensity, or exercise selection.

Other situations may make recovery more appropriate.

Significant or persistent nausea, vomiting, dehydration, dizziness, inability to maintain adequate nutrition, or other concerning symptoms should be discussed with your healthcare provider.
